What are Home Care Packages?

The Australian Goverment and Department of Health and Aged Care has introduced a specialised funding program called Home Care Packages to provide support to older people with complex needs to help them stay at home. 

These packages include a variety of services such as:

  • help with household tasks
  • equipment (such as walking frames)
  • minor home modifications
  • personal care
  • clinical care such as nursing, allied health and physiotherapy services.

and are aimed at assisting with daily activities and also covering essential items to enhance safety at home.

Am I eligible for a Home Care Package?

If you’re an older person and want to find out if you are eligible, go to about Home Care Packages on the My Aged Care website.

To find out if you’re eligible for government-funded aged care services, you can apply for an assessment online.

What are the steps to get home care physiotherapy?

If you are approved for a Home Care Package, your home care case manager will help you coordinate and organise support for you based on your needs. This includes getting a referral for Physiotherapy.

We will obtain the HCP case manager details and call them on your behalf to introduce our services and get approval.

Once we receive approval from your home care case manager, our friendly physiotherapists will work with you to achieve the best health results and meet your personal goals.

DO I NEED TO PAY FOR HOME CARE PHYSIOTHERAPY?

Hampton Park Physiotherapy directly bills the HCP company for any services provided. There is no gap payment for your physiotherapy service.
